Embrace Aging Your Own Way

Blue Ridge Independence at Home is a Program of All-Inclusive Care for the Elderly (PACE). It’s an integrated new healthcare solution for seniors in our service area who need assistance but want to remain independent and continue living at home. Although Blue Ridge Independence at Home was developed out of Blue Ridge Hospice, your trusted community serious illness and end-of-life care provider since 1981, the care our program participants receive through Blue Ridge Independence at Home is not hospice care. Independence at Home provides and coordinates the care you need while staying safe and happy at home.

How We Work

  • All-Inclusive Care: Our program offers a wide range of services unlike most other healthcare solutions, with the goal of providing any and all care a participant may need. Care is provided by providers on our interdisciplinary team at the Clinic, or coordinated with providers in our extensive care network.
  • Customized Care Plans: We take on the task of coordinating the care our participants need every step of the way, as to ensure health needs are met and to alleviate the complexities of navigating the fragmented healthcare system we live with in our country today.
  • Focus on preventative and long-term care: Emphasis is placed on preventative care and long-term management of chronic conditions, which can lead to improved health outcomes and quality of life.
  • Community-Based Care: The goal is to help participants, who are eligible for nursing home care, to remain at home and living in our communities instead.
